Initial Screening
Our team will review basic eligibility, including age, income, and current housing situation.

Intake Conversation
Qualified applicants may be invited to speak with our team to discuss needs, expectations, and program guidelines.

Placement Decision
If approved, placement will be offered based on availability and program fit. If space is not immediately available, applicants may be placed on a waiting list.

After completing the intake application, please download the form to your device and submit it by email to Welcome@heritagehomesteadfoundation.co.site for review. Applications will not be processed until the completed form has been received.
